Homeless after leaving hospital

This page provides advice for people who will not have anywhere to live after they have been discharged from hospital, including if you are unable to leave hospital as you require specialist housing.

If you believe you will not have anywhere to live after leaving hospital, either because you do not have a home or because your home is now unsuitable for you to live in, you should tell the nursing staff about your situation as soon as possible. They will be able to make your situation known to the hospital discharge team for advice.   You should also be referred to the Housing Options Service - you can contact us directly on 01623 457252.

If you have a home already, but it needs to be adapted to suit your needs, you should be referred to the Nottinghamshire County Council Occupational Therapy Team - you can contact them directly on 0300 500 8080. Find out more about how your home can be adapted here
