7. We have a limited number of tables and gazebos, please indicate if you will need a table or gazebo and we will do our best to accommodate this. Please indicate if you will be bringing your own tables or gazebo. Please bring your own chairs.

10. If you are able to provide an activity without charge, that is fantastic and thank you so much for your support. If there are any costs associated with your activity, please indicate this and we will be in contact to confirm if we are able to provide the funds.

11. How many people will you be bringing with you on the day?

12. Do you have any volunteers supporting on the day. If so, how many?

13. Do you have public liability insurance? Organisations are required to hold a current policy of insurance in respect of public liability or third party risks. The relevant limit of indemnity shall be an amount (not less than £5 million), approved by Ashfield District Council's risk and insurance section. If you are not part of an organisation and don't have public liability insurance we will be in contact to discuss options, you may be able to provide an activity under our Ashfield District Council - Mill Waters volunteer policy.
